Dialysis Nurse
Hemodialysis:
1. Provide safe and effective delivery of care to patients with ESRD.
2. Responsible for accurate implementation of treatment prescription to ensure stable treatment therapy as indicated.
3. Assess patient’s response to hemodialysis treatment therapy making appropriate adjustments and modifications to the treatment plan as indicated by the prescribing physician
4. Communicate problems or concerns to the Clinical Manager or Physician.
5. Initiate initial nursing assessment and ongoing evaluation and documentation of patient care needs according to the institution’s policies.
6. Actively participate in the pre-evaluation, initiation, monitoring, termination, access homeostasis, and post evaluation of patients receiving hemodialysis treatment therapy according to established procedures.
7. Provide, supervise, and monitor hemodialysis access care according to established procedures.
8. Responsible for the implementation, administration, monitoring and documentation of patient’s response to prescribed interdialytic transfusions, including appropriate notifications of adverse reactions to physician and appropriate blood supplier.
9. Ensure accurate documentation on the hemodialysis treatment sheet.
10. Maintain overall shift operation in a safe, efficient, and effective manner.
11. Act as a resource for other staff members.
12. Collaborate and communicate with physicians and other member of healthcare team to interpret, adjust, and coordinate care provided to the patient.

Peritoneal Dialysis:
Follow and strictly implement NKTI PD Unit policies, procedures and standards regarding the performance of PD & exit site care.
Make proper documentation and recording of PD flow sheet.
Assess patency of PD catheter.
Assess dialysate for clarity.
Perform PD exchanges on all patients outside the PD unit 24 hours a day as needed, Monday – Sunday.
Shift patients from one type of PD schedule to another (Ex. CIPD to CAPD).
Secure all training supplies and equipment for the performance of PD.
Do after care and safekeeping of cycler machines.
Inform PD unit of status and need for repairs of cycler machines after each use.
Monitor patients on dialysis and refer immediately to PD nurse for any PD-related problems.
Do exit site care when starting and terminating PD, shifting from one type of PD schedule to another, and when PD leaks are diagnosed.
Monitor training checklist for consistency.
Respond immediately to referrals to attend to PD patients from PD and ER nurses, nephro fellows, and nephro consultants during the day and at nighttime as necessary.
Inform PD nurse for any supplies/equipment that needs to be changed to the patient.
Administer IP (intra peritoneal) medications as needed, supervised by PD nurse.
